India’s Comptroller and Auditor General conducted an audit of India’s digital ID system, which over 99% of the population is enrolled in. The audit found 475,000 had the same biometric data but were assigned to different people and the security practices of vendors and service providers were very poor, leaving the system wide open to abuse.

Analyst Comment: I get heat every time I point out the massive government incompetence baked into the system. A lot of people simply have trouble reconciling the idea that the government and elites engage in conspiracies like plans to implement digital ID systems, but often lack the competence to implement them without massive issues. Most government and elite-created conspiracies will fail miserably.
